If you’ve changed your personal information (name, address, telephone number, email address, employment etc) please complete this online form and we’ll update our records.
Direct Debit Form* If you’ve changed your bank account or need to complete a new direct debit form, please download and complete the above form and return it to us to the address or fax number shown on the form.
